Work Request / Authorization
IMPORTANT: By filling out this form you are agreeing that you have read and understand our terms below:
- Payment is due to Lanier Aircraft Maintenance (LAM) upon completion of work unless prior arrangement is made.
- A 3% fee is automatically added to the invoice total to cover shop supplies.
- A 3.5 % fee may be added to all invoices paid by Visa/Master card.
- No fees for invoices paid by check, cash or on account.
- All estimates are based on what we can see externally. Increased cost may be incurred after disassembly of component or after deeper inspection. Owner will be notified of increased cost if requested.
- All estimates do not include taxes if applicable.
- All estimates, unless stated, do not include shipping charges.
- Core returns may be subject to additional charges if core is rejected by the repair facility. This charge may occur after invoice paid. A supplemental invoice will be sent if core is rejected.
- Customer supplied parts will incur a 10% surcharge based on retail cost of part.
- 100% deposit required on parts total exceeding $1000.
- Aircraft left over 30 days will be charged $25.00 per day storage fee after 30 days.
- Invoices unpaid after 30 days are subject to a Mechanics Lien filed against the aircraft.
- A 2% per month service fee will be applied to all outstanding balances after 30 days
WARRANTY INFORMATION:
LAM does not warranty labor or parts for customer-supplied parts. The warranty will be void if other facilities or mechanics work on the aircraft after it leaves LAM.
Warranty work must be claimed within 90 days or 50 hrs whichever comes first.
I, as owner and/or authorized representative/agent of the owner of the aircraft described herein, hereby authorize Lanier Aircraft Maintenance (LAM), its agent(s) and employee(s), to undertake and perform all repairs detailed herein, together with all necessary labor, parts, materials, and supplies required in order to effectuate same. LAM, its agent(s), and employee(s) is/are further authorized to operate and/or test fly said aircraft for the purpose of testing and inspection. I understand certain repairs and maintenance will be required to obtain an airworthy log entry and hereby authorize all repairs, maintenance, parts, materials, supplies, addition to those listed above, which may be deemed necessary by LAM. I understand and agree that all labor, parts, materials, supplies and work are performed on a COD basis unless other arrangements have been agreed upon. In the event payment is not made upon delivery, same could be subject to finance charges and parking and/or storage fees, which I agree to pay. In order to secure payment for all such parts, labor, materials, supplies, and work, LAM is hereby granted an Express Mechanics’ Lien. I the event it becomes necessary for LAM to employ the services of an attorney to enforce its rights hereunder to collect any balance due, the owner and/or agent agrees to pay all court costs and reasonable attorneys’ fees incurred with regard thereto by LAM. LAM is relieved of all responsibility for all loss or damage to said aircraft or articles left therein in the event of fire, theft, or any other cause beyond the direct control of LAM. By signing below, I attest that I am the owner and/or authorized agent, of the owner, of the aircraft, and have full authority to approve the work described on the previous page.
